Social Services

The Social Services team constitutes a core home health and hospice discipline. Social Workers act as health care advocates for patients and their families during the course of illness. Social Work services include, but are not limited to, the following:

  1. Conducting thorough assessments of patients and their families, focusing on physical, social, emotional, psychological and financial factors which may affect a patient's course of illness.
  2. Providing counseling and support to patients and their families.
  3. Determining financial needs and helping patients procure available financial assistance.
  4. Providing information and access to community resources as needed.
  5. Educating caregivers about relevant issues, including management of stress and relaxation techniques.
  6. Providing bereavement counseling and support for family members, friends, or significant others.
  7. Educating patients and families about long term care options and helping to design a long term care plan.
  8. Providing information on legal issues and connecting families with legal assistance.
  9. Guiding families in funeral planning.
